Forgetting a document at the DMV means rebooking your appointment, often weeks out. Here is the universal checklist that works in all 50 states, plus the most common reasons test-takers get sent home.

Documents (originals, not photocopies)

  • Proof of identity — US passport, certified birth certificate (with raised seal), or permanent resident card.
  • Proof of Social Security number — SSN card, W-2, paystub, or SSA-1099.
  • Proof of state residency × 2 — utility bill, mortgage statement, lease, bank statement, or insurance card. Both must show your current address.
  • Proof of name change (if applicable) — marriage certificate, divorce decree, or court order.

If you're upgrading to REAL ID, all 4 categories are required. For a non-REAL ID renewal, requirements are usually less strict, but bring everything anyway.

Payment

  • Cash, check, or card — varies by office. Some smaller DPS/DMV offices don't take credit cards.

For the written test specifically

For minors taking the permit test

  • Parental consent form signed in front of a notary or DMV staff (depending on state).
  • Driver education completion certificate (required in ~30 states for under-18 applicants).
  • Parent or legal guardian present in some states (e.g., NJ, GA require parent signature at the counter).

What gets you turned away

The 5 most common rejection reasons:

  1. Photocopies instead of originals. Birth certificates and SSN cards must be originals or certified copies. iPhone photos don't count.
  2. Address mismatch. Your two residency docs must show the same address as your application form. Got an old utility bill? Bring a recent one.
  3. Expired ID. A passport that expired last month is not valid identity proof.
  4. Wrong vehicle for road test. If you booked a road test, the car you bring must be insured, registered, and roadworthy. No trip to the road test in your friend's expired-tag pickup.
  5. Late arrival. Most DMVs have a 5-10 minute grace window. Past that you're rebooked. Plan to arrive 20 minutes early.

What you do not need to bring

  • A printout of your appointment confirmation (most DMVs lookup by name/SSN now).
